GETTING READY FOR YOUR VISIT

Patient Information for Mountain's Edge Families

Walking into a dental office for the first time, or even the tenth time, goes a lot smoother when you know what to expect. We put together this page so you can review our forms, understand how we handle insurance, and get a feel for our office policies before you ever sit in the chair.

Whether you are a new patient in Mountain's Edge scheduling your first checkup or a returning patient preparing for a procedure, you should be able to find practical answers here without having to call us first. And if you can't, our front desk team is always happy to walk you through it.

BEFORE YOU ARRIVE

What to Have Ready for Your Appointment

A little preparation ahead of time means less paperwork in the waiting room and more time spent actually getting to know your care team.

New Patient Forms

We will email or text you a link to complete your health history and registration forms ahead of time. Filling these out at home saves you time and lets us review your information before your first visit.

ID and Insurance Card

Bring a valid photo ID and your dental insurance card so our front desk can verify your coverage before treatment and give you an accurate estimate of costs.

Medical and Dental History

A list of current medications, allergies, and any recent dental work or hospitalizations helps us treat you more safely, especially if you are seeing us for the first time.

Insurance, Billing, and Office Policies

We work with most major PPO dental insurance plans and will file claims on your behalf whenever possible. Our team verifies your benefits before major treatment so there are no surprises when the bill arrives. If you do not have dental insurance, we are glad to talk through payment options at your visit.

We understand that dental visits can bring up questions about cost and coverage, so we try to be upfront about both from the very first phone call. Here is what you can count on from us:

  • Insurance verification before treatment: We confirm your plan details and explain your out-of-pocket responsibility before we begin any procedure.
  • Flexible payment arrangements: We accept major credit cards, HSA and FSA cards, and can discuss financing options for larger treatment plans.
  • Transparent treatment estimates: You will receive a written estimate before we move forward, so you know the cost ahead of time, not after.
  • 24-hour cancellation notice: We ask for at least one business day's notice if you need to reschedule, which lets us offer that time to another patient who needs it.
  • On-time appointments: We respect your schedule and do our best to seat you close to your appointment time, with minimal waiting.

Our office hours vary by day, so it is worth checking before you plan a visit: Monday: 8:00am – 5:00pm* Tuesday: Closed Wednesday: 8:00am – 5:00pm Thursday: 8:00am – 5:00pm Friday: 8:00am – 3:00pm Saturday: 9:00am – 3:00pm Sunday: 9:00am – 3:00pm *Every Other Monday. YOUR FIRST VISIT

What Happens When You Arrive

If this is your first visit with us, plan to arrive about 15 minutes early so our front desk team can confirm your paperwork and insurance details without rushing. A typical new patient appointment includes a full exam, digital X-rays if needed, a cleaning, and a conversation with your doctor about any findings or concerns.

We will never pressure you into treatment during your first visit. Instead, we explain what we see, answer your questions, and give you time to decide what makes sense for your schedule and budget. Returning patients can expect the same unhurried approach at every routine checkup.

Frequently Asked Questions

Common questions from Mountain's Edge patients about forms, insurance, and office policies.

Do I need to fill out paperwork before my appointment?

Yes, and we make it easy. New patients receive a link to complete health history and registration forms online before their visit. If you are unable to complete them ahead of time, just arrive 15 minutes early and our front desk team will help you finish them in the office.

Which dental insurance plans does Elevate Dental accept?

We accept most major PPO dental insurance plans and will verify your benefits before treatment so you know what to expect. If you are not sure whether your plan is accepted, call us at (702) 577-3702 with your insurance details and we will check for you before you schedule.

What if I don't have dental insurance?

Many of our patients do not have dental insurance, and that is completely fine. We offer transparent, written treatment estimates up front, accept credit cards and HSA or FSA cards, and can discuss financing options for larger treatment plans so cost does not get in the way of care.

What is your cancellation and rescheduling policy?

We ask for at least 24 hours' notice if you need to cancel or reschedule an appointment. This gives us the chance to offer that time slot to another patient who may be waiting. Repeated last-minute cancellations may result in a scheduling fee, but we always try to work with you if something comes up unexpectedly.

How early should I arrive for my appointment?

New patients should arrive about 15 minutes early to complete any remaining paperwork and confirm insurance details. Returning patients can typically arrive right on time, though arriving a few minutes early is always appreciated, especially if you need to update your contact or insurance information.

What should I bring to my first appointment?

Please bring a valid photo ID, your dental insurance card if you have one, and a list of any current medications or allergies. If you have had recent X-rays taken at another office, let us know ahead of time and we can request them so you do not need to repeat them unnecessarily.

Can I bring my children to their own appointment, or do I need to stay?

A parent or legal guardian must accompany any patient under 18 to their first visit so we can review health history and discuss treatment. For later visits, we are happy to work with each family's preference, whether that means staying in the room or waiting up front.

How do I request my dental records or X-rays?

Call our office at (702) 577-3702 and our team will walk you through a simple records release form. Once it is signed, we can send your records or X-rays directly to another dental office or provide a copy for your own files, usually within a few business days.
